Prior to Phantom Pain

During the events of Ground Zeroes, Big Boss embarks on a mission to rescue Paz after discovering she had survived the battle at the end of Peace Walker. Chico is also discovered to have been captured after he himself attempts to rescue Paz after hearing about her discovery. In the meantime, an nuclear inspection is going to take place back at mother base which was allowed by Heuy Emmerich.

So Snake now has to rescue Chico and Paz from Camp Omega. After rescuing Chico and Paz, Big Boss then heads back to Mother Base, soon he discovers that Paz has a bomb implanted in her and orders the medic on board to remove it. After successfully removing the bomb, Big Boss continues on his way back to mother base, Only to discover it is being attack by an unknown enemy force that used the nuclear inspection as a smokescreen to enter mother base.

After rescuing few men and his second in command, Kazuhira Miller, Snake retreats from the now destroyed mother base. Soon when Paz regains consciousness, gets on her feet and opens the door of the helicopter, telling Snake that there was a second bomb implanted in her. As she jumped, Snake attempted to stop her but was unable to, as she exploded mid-drop and the medic jumped in front of Big Boss to cover him from the explosion, soon the helicopter begins losing control and collides with a pursuing enemy chopper. Soon Big Boss, Miller, and an third unknown person are sent to a hospital where Snake falls into a coma for 9 years and thus, begins the events of The Phantom Pain.

The story deals with Big Boss' quest for revenge on those who destroyed Mother Base, along with the foundation of the "Diamond Dogs".

hospital Raid

Snake awakens from his coma, looking around as nurses are managing the room. As one of the nurses is about to leave, Snake groan and catches the nurse's attention. She gets closer to Snake to inspect him and is shocked, then she runs out of the room, screaming for the doctor. Snake then loses consciousness again. A few days later Snake awakens to the doctors voice, who is instructing Snake to move his head around. Snake does as he says and then the doctor proceeds to explain that Snake has been in a coma for 9 years. Snake begins to go into a panic and the doctor orders the nurse to inject Snake with a sedative. Snake falls into slumber once again. Several more days pass and Snake wakes up once again to the doctor standing in front of him with a board set up. The doctor begins telling Snake about his injuries he received in the accident, saying there were over 100 foreign bodies in him, most of them being removed. The doctor then says that there are shrapnel that they cannot remove, one being by Snake's heart and the other being on his forehead. The doctor then places an X-ray of Snake's upper body and telling Snake to look down. Snake does so, noticing his left arm is missing and begins panicking again. The doctor calls for the nurse and she injects Snake again with a sedative, causing Snake to fall unconscious.

A week passes and Snake awakens to the nurse and doctor arguing in another language and the doctor then approaches snake, stating that he shouldn't have awoken and now his face must be changed immediately. After some time, the surgery was complete then the doctor begins removing Snake's bandages and remarks that his face has healed nicely. The doctor then tells Snake that as of now, his name is "Ahab". A mysterious figure then appears into the room, grabbing the Nurse standing behind the doctor and begins choking her her, eventually killing her. The doctor then grabs a photo of Snake from 9 years ago and shows Snake the old photos. The doctor puts the photos down and grabs a mirror to show Snake one final look at himself. As the doctor is holding the mirror, the mysterious figure then begins choking him too. The doctor attempts to fight, pulling out his gun, but he kicks Snake from the bed and as Snake is on the floor, the doctor is killed and drops his weapon, landing in front of Snake. The assassin walks away and makes a call, saying that a patient in another bed saw her. She hangs up and begins slowly walking towards Snake and notices him reaching out for the handgun. She picks up the weapon and aims it at Snake before being assaulted by a bandaged man. She throws him off, pulls out a knife and begins walking towards Snake and the other patient. The bandaged man throws objects at her, including a jar of alcohol which breaks and the assassin is covered in it. The assassin throws a knife at the man, hitting his right shoulder. The assassin then makes her way to Snake and grabs him. She lifts him up to his bed and begins choking him. Just before she can kill him, the bandaged man finds a light and throws it onto her. The assassin bursts into flames from the alcohol that was on her and she starts screaming in pain as she moves across the room. The assassin makes one more attempt at Snakes life, but the bandaged man finds another jar of alcohol and throws it at her, causing more flames to erupt from her. The assassin then jumps out of the window, presumably dying. Snake lifts his arm and sees he now has a prosthetic limb as white petals are gliding through the air. The Bandaged man then goes on to say to say it is time to go, calling Snake Ahab.

The man reveals himself to be Ishmael and that he has been watching Big Boss during his coma. Injecting Big Boss with a dose of Digoxin, in the hopes of helping him move. Ishmael proceeds to lead Big Boss through the hospital, but not before coming across the Third Child, who floats through the ceiling and after he disappears, flames appear in the elevator and an explosion occurs, throwing Big Boss and Ishmael back. Big Boss' arm was also dislocated when he landed on the ground. The Man On Fire suddenly appears from the flames, making his way towards Big Boss, but is driven away when the fire Suppression system is activated, with the man seemingly evaporated.

Big Boss and Ismael regain their composer and continue down another way out of the hospital. Ishmael spends the next 20 minutes teaching Big Boss the basics of stealth and on-field medical care, fixing Big Boss' arm in the process. Sneaking past masked men, who were killing anyone residing in the hospital, in the search for Big Boss, the duo are eventually found, but saved due the Man On Fire's, sudden appearance. Driving the man away with the use of the sprinkler system, Ishmael and Big Boss continue their trek through the hospital. Making their way to the hospital entrance, Ishmael distracts the masked men in the room, giving Big Boss a chance to sneak out, but not before another appearance by the Man On Fire and the Third Child, who begins attacking the masked soldiers. Trying to escape, big boss is saved, when an armored vehicle crashes into the building and charges at the Man On Fire in an attempt to kill him. Using the chance, Big Boss escapes and regroups with Ishmael, who is now driving an ambulance. The two escape, only to be attacked by debris launched by the Man On Fire. The vehicle is forced off the road and the the two are knocked out in the ensuing crash.

Big Boss awakes some time later, only to find that Ishmael is now gone. Crawling his way out of the vehicle, Big Boss is greeted by the chopper from before and the Third Child. Looking back at the chopper, the Third Child raises one of his arms and the chopper is destroyed by a massive whale on fire. After the whale swallows the helicopter, the Man on Fire appears on a horse covered in flames. Big Boss regains his composer and begins to run, only to be stopped by Ocelot on a horse. Claiming to be a friend and with no other choice, Big Boss grabs his arm and hoists himself onto the horse. Seeing the Man On Fire, Ocelot spurs his horse to move. Barreling through the forest to escape, Ocelot hands Big Boss a shotgun to hold off the flaming horse chasing after them. After an intense chase, the two seem to escape only for the bridge they were crossing to be struck by lightning, causing the two to fall below. Ocelot helps Big Boss out of the riverbed and leads him out onto the other side of the bridge.

Helping him onto the horse, Ocelot and Big Boss ride off, while Ocelot explains whats been happening during the nine years that Snake was in a coma. He had been given two missions. The first was to get Big Boss out of the Hospital raid. The second was to rescue the man who gave him the first. Kazuhira Miller had been recently captured with his men killed in Afghanistan. They don't have much time, so Big Boss spends the next few weeks developing his motor skills, before Ocelot equips him with a relatively new prosthetic arm.

Afghanistan

After the first mission: Phantom Limbs, Metal Gear Solid V: The Phantom Pain's story becomes non-linear. Some story related missions can be played out of order and however the player does so, thus there is no accurate interpretation of this story, just another way the player can experience it.

Several weeks after the hospital escape, Big Boss and Ocelot make it to  Afghanistan. Ocelot then explains to the now titled Punished "Venom" Snake that Miller is being held captive at Da Gwhandai Khar. After explaining to Snake the basics of gathering intel, he then leaves Snake to begin the mission. After reaching Da Gwhandai Khar, Snake finds Miller handcuffed to a pipe, noticing that he has lost his right arm and his left leg. Snake then removes Miller's handcuffs and rescues him from the outpost. Soon the two are attacked by "The Skull Unit' and and begin riding away from them on D-horse.

After losing them, Snake brings Miller into another landing zone and is picked up by Pequod. As Pequod takes Snake and Kaz back to their mother base, Miller begins talking to Snake about the events of Ground Zeroes and what happened afterwards during Snakes 9 year coma, explaining that he has been doing dirty jobs to bring in money and even began to train rebel soldiers in Afghanistan before being captured by the soviets. Arriving at the new Mother Base, Kaz makes his intentions clear that his only intent is to gain revenge on the organization known as cipher, for what they did to them 9 years prior. After Kaz is taken for medical treatment, Ocelot gives Snake the Fulton extraction device and teaches him in its use. As Snake completes his mission, gain intel on the troops in Afghanistan, he is eventually given a mission to rescue a Soviet engineer, the person who made Snakes Bionic arm, who attempted to defect. Sneaking into Wakh Sind Barracks Snake successfully rescues the engineer, who proceeds to join Snake's R&D team, giving Snake new options for his arm.

Later on snake meets a lone wolf pup, with a missing eye. Snake Fultons it back to base, only to be greeted by said pup when he returns. Ocelot is there and explains that the pup has been named DD. Ocelot promises that he will train and core for the pup, and that he might be useful when he gets bigger.

Snake is then given a mission by the CIA to retrieve a special new weapon, called the Honey Bee, that was supplied to the local Mujahideen to help fight off the Soviet control. However before the weapon could be put to use, the Hamid Mujahideen, were wiped out without a struggle, thought to be causing by some biochemical weapon, despite claims to contrary by the Soviets. Snake begins the mission east of the presumed location of the Honey Bee. Snake makes his way through the nearby mountain relay base, in the process rescues that last Hamid Mujahideen left alive. The man later known as Silent Basilisk, is mute, but does his best to help Snake find the Honey Bee. After extracting the last Hamid, Snake, continues his way to the fort.

Infiltrating it and finding the Honey Bee, only to be caught off gaurd by a sudden mist. The boy in the mask appears and floats past snake into the mist, where a massive mechanical arm reaches out and grabs Snake. Now held upside down by a massive machine, Snake finally comes face to face with the man that led the attack on Mother Base 9 years ago. Skullface as he is known, tells Snake that there is a bigger picture, he is not seeing. The man explains that it doesn't matter for he intends for Snake to die here. Snake begins to notice the Skulls in the area, taking control of the Soldiers there. The machine lets go of Snake and picks up Skullface, carrying him off into the mist, as Snake prepares to escape the skulls. Sprinting through the mist, Snake barely avoids the skulls, making distance and getting to relative safety. Calling in Pequod, Snake exfiltrates the hot zone. On the flight out Snake and Miller can only guess the answers to the mysteries that have come to light. There only conclusion is that they must all be connected to cipher.

Snake continues doing missions, collecting GMP, soldiers. and clues to where Cipher could be. He is eventually given a mission to rescue one of the few Mujahideen left. Malak as he is known has been captured and being held at Lamar Khaate Palace. The mission was given by Malak's father before he died. Miller explains that after he was captured, his home village was wiped out by the soviet's scorched earth operation. Snake makes his way to the palace, only to find Malak getting ready to be transported to the fort north of the palace. While tracking Malak to the fort Snaker overhears the driver talking with different soldiers along the way. He learns that not only that the Soviets did not burn down that village, they were in fact trying to figure out who did as well. He also learns that a plague had spread to the village shortly before it had burned.

These tales are confirmed when Snake manages to rescue Malak and extract him via chopper. Malak says that not only has he never heard of the client, but that he village had long burned to the ground before he was captured and that he has sen strange men snooping around the burnt village before he was captured. Miller suspects them to be apart of XOF the unit led by Skullface. Miller decides that it is best to tell the "client" that Malak died of his injuries and keep Malak safe in Mother Base.

He then encounters a sniper when heading through the Aabe Shifap Ruins. Ocelot informs Snake that a sniper by the name "Quiet" has been killing soldiers throughout the area and to beware. A sniper battle ensues, with Snake emerging as victor. Snake then approaches the sniper "Quiet" lying on the ground and points a gun at her. Miller and Ocelot then dispute about what Snake should do with her life. Snake then puts away his weapon and prevents Quiet from committing suicide and handcuffs her (The player also has the choice to kill her and doing so will result in her disintegrating as she dies.). A helicopter is then called in and Snake puts Quiet inside with Miller protesting over the radio. As the helicopter is leaving, Snake puts Quiet on the bench and as soon as he turns around, he notices Quiet quickly "escaping" from the helicopter, with Ocelot feeling sympathy saying "she's one of a kind." and Miller answering they didn't need her type.

As Snake is heading back to mother base, the pilot notices that a jet is after them and attempts to shake it off. The fighter jet then shoots heat guided missile, but the pilot shoots out several flares, sending the missile towards the flares. The jet goes around Snake's chopper and shoots another missile, but the flares don't work this time. Snake attempts to use the mini-gun attachment on the chopper but falls over since the helicopter is shaking. Snake attempts to use the mini-gun again, but sees that something else has grabbed it and Quiet then reappears and shoots the missile, destroying it. Snake then gets back on his feet and Quiet goes back and grabs a Sniper Rifle that was laying inside the chopper. Snake begins shooting at the jet as it begins heading towards them and Quiet aims at it, then shoots, successfully hitting the jet's pilot in the head, rescuing everyone on board the chopper. She sits back down on the bench, unloads the rifle, and places her handcuffs back on.

Arriving back at mother base, Snake see two other Helicopters escorting Snake's, with snipers trained on them. Being directed over to a helipad with gunmen waiting, Quiet jumps off the chopper and floats down to the surface, turning invisible in mid air and landing with a thud. After demonstrating her abilities once more, she is surrounded by men with rifles trained on her. Miller orders them to shoot, but Ocelot intervenes, saying that she saved the boss. Miller commands them to shoot only for the Snake to come in commanding them to lower their arms. He tells them to put her in a cell. After assuring her, Quiet goes with the men willingly and Snake assures Miller that eventually they will have to kill her. Miller and Ocelot walk off, leaving Snake to contemplate their new guest.

While out on the field Snake is contacted by Miller and told of Dr. Emmerich being in the area. Huey had attempted top make contact, trying to defect from Skull Face. Huey is suspected to have had a hand in the destruction of MSF. Snake is given the mission to find Huey and bring him back. Snake makes his way to the Zerak Power Plant. Moving past the facility, Snake picks the lock to one of the gate doors leading onto the nearby rock face. Coming up into a hanger, Snake sees a new bipedal weapon currently in development.

Before he can investigate further, he spots Huey, who is now standing due to a mechanical augmentation he developed, arguing with Skull Face. Huey is explaining that the weapon isn't finished, with Skull face showing little concern. One of his men whispers to him that Huey has been in contact with Big Boss. Skull Face does not take the news well, throwing Huey down a flight of steps. Skull Face has his men bring Huey back to "the base". One of Skull Face's men picks Huey up using a walker gear with a mechanical arm and takes him to the base. Getting into a jeep, Skull Face heads off with his men following. Snake uses the opportunity to collect intel on Huey's location.

Finding documents, Snake discovers that Huey is at a lab at the Afghanistan Central Base Camp, Snake sneaks his way out of the of the power plant and makes his way to the base camp. The soldiers, now equipped with new walker gears, patrol the area. Snake manages to find the laboratory that Huey is in. Entering the building, Snake is greeted by a disturbing sight. The Mammal pod from the peace walker incident. Still functioning, speaking in the Boss' voice. The Mammal pod is confused as to who is standing before it. Huey calls over, assuring that's it's just a machine. Surprised to see Snake again, Huey is taken aback just before snake puts a bag on his head. Before Snake can take him, Huey insists on using his custom walker gear to make the process easier. Snake complies, hoisting Huey up to turn on the machine, before taking control himself. Escaping the facility, Snake is stopped before he can reach the chopper, as the massive bipedal weapon he has seen before comes crashing down from the sky, now standing upright.

As the dust clears away, the Third Child is seen levitating in front of the mech. Soon, it is revealed that the bipedal weapon is Sahelanthropus, the Metal Gear that Huey was responsible for creating. Sahelanthropus extends its hand and skull face is shown standing on it, calling Huey useless then exclaiming that this is the real Sahelanthropus and that weapons will learn to walk upright. Sahelanthropus then puts Skull Face into a Helicopter and he leaves the area, leaving Sahelanthropus to kill Snake and Huey. Snake begins fleeing from Sahelanthropus, making it to Pequod and exfiltrating from the area. Sahelanthropus attempts to take down the chopper but is stopped by a barrage of fire from the choppers' mini-gun. Snake then escorts Huey to mother base.

Now at Mother Base, Huey is under interrogation. Accused of helping Skull Face with the attack on MSF 9 years agos. Huey, claims that despite his insistence on the inspection of mother base, that it was only for MSFs best interests. When questioned as to why he not only survived, but was now found working with Skull Face. After listening to Huey's claims of being captured into servitude over that last 9 years, Ocelot and Miller head into the back room where Snake is waiting. The group decides to put Hueys skills to use in the meantime, while they figure out his true allegiances.

Africa

Snake now heads for Africa, based on the info that Huey gave. The new ST-84 is not enough to change the tides of the world. XOF is researching a new weapon. He is contacted by Miller, who explains that a NGO has asked them to blow up a oil facility in the hopes of stopping the oil spills that are polluting the land. He further explains that the militia group is being armed by someone from the U.S. and presumes it to be Cipher. Landing in Angola, Snake begins his trek to the oilfield. Along the way he comes across two startling relations. Child soldiers are being deployed by the militia and that there is another burnt down village, like the one in Afghanistan. Finding his way to the oil facility, Snake infiltrates, sneaking past the posted sentries and plants C4 on the oil separator tank. Before setting it off he makes his way to the oil transfer pump and shuts it down. Once shut down Snake's sees what the oil has been hiding. Bodies, covered in tumors rise from the oil, causing Miller to be disturbed. Setting off the C4, the militiamen who where patrolling outside of the Mfinda Oil Field become alerted and rush back to seal off the facility. Snake manages to escape the facility during the sealing. Back on the ACC Miller and Snake go over what they have found. After some research, Miller finds that company that ran the facility are the ones behind the spill and that even they are not in full control. The company, SANR is just a front for a shadow running them from behind the scenes, which he suspects can only be Cipher.

Snake and Ocelot discover that the Walker Gears Huey has helped developed has began appearing throughout the black market. Ocelot worries about their numbers and becomes suspicious that Cipher could be behind the leak. Diamond Dogs is then contracted by the MPLA (People's Movement for the Liberation of Angola) to eliminate the Walker Gears that have been assigned by a PF force at the Ditadi Abandoned Village. Miller uses this job as a way to get closer to the force's supplier, Cipher. After eliminating the Walker Gears, Miller discovers that there was a call for resupply for the Walker Gears from the company that "owned" the Mfinda Oilfield, SANR, confirming Miller's suspicion of Cipher being the one supplying the tech to the Private Forces.

Snake comes across intel that a Private Force is transporting cargo as payment to Cipher. Snake then goes onto steal the cargo from the Private Force. Snake follows the armored vehicles that are to escort the cargo truck to Nova Braga Airport. As Snake goes near the truck, the area is suddenly covered in mist and several Skulls jump from the truck and begin attacking Snake, Snake then manages to escape from the Skulls with the cargo truck and returns to the ACC. Miller contacts Snake and tells him what they discovered on inside the truck. They found drums of malachite inside the truck along with YellowCake, a material used for nuclear weapons, though Miller is confused since there is too little to make a nuclear warhead. Miller tells Snake that he'll look more into it to see why it was important.

Snake is later contacted to take out six Mbele soldiers who have been captured. Snake makes his way to Kungenga Mine where they are being held. After naturalizing the first target, Snake moves into the mine to find the other five, only to come across 5 children locked in a cell. The children, scared, try to bribe Snake with Diamonds, for their lives. Miller calls, telling Snake there they have only two options, Heaven or Hell. Snake says there's another, Miller then mentioning Outer Heaven. With a recording going, Snake shoots past the children to scare them, making the audio sound as if they were all shot, so the client won't know any better. Snake then breaks the kids out of the cell, taking them with him. With children on his tail, Snake manages to guide them past enemy soldiers and to the Landing zone. Arriving back at mother base, Miller, after a short test, has the kids escorted to a quarters that has been prepared for them. Miller then talks with Snake about giving the kids a chance at a real life, giving them education, and teaching them to do basic jobs, mentioning there is no room for angels in their heaven.

Snake returns to Africa to retrieve Shabani, the leader of the child soldiers Snake recently rescued from the mines. Miller informs Snake that Shabani was taken to the Ngumba Industrial Zone, otherwise known as "The Devil's House" (Nzo ya Badiabulu) as called by the locals. The child soldiers contracted Diamond Dogs on this mission, offering the diamonds they hid as payment. Snake lands by the Munoko ya Nioka Station and makes his way, eventually reaching a broken bridge by a valley. Snake finds an alternate route through the bottom and sneaks pass enemies, leading to a tunnel that is another way of entering Shabani's location. Snake enters the building and discovers there are human subjects in bed, with microphones attached to their throats. Snake pulls an earphone out of man's throat and hears another language being spoken. Snake continues making his way through the disturbing sight of rows and rows of patients, all with symptoms similar to the bodies that rose up during the Mfinda Oil Facility incident, with large bumps appearing where their lungs are at.

Snake finds Shabani and begins cutting him free from his restraints. As Snake cuts the restraints, he is surprised by the sudden appearance of Skull Face, who was talking to another patient in their language. Skull Face goes on to say "rest in peace" and pulls out his weapon and shoots the patient. Shabani begins panicking, with Snake trying to keep Shabani quiet. Skull Face catches Snake and points his weapon at him. Skull Face realizes it's Snake and puts his gun away, going on to tell Snake to "Burn with the rest of them". The third Child appears and as Skull Face walks away, The Man on Fire appears out of nowhere and assaults Snake. The Man on Fire freezes however, as the Third Child is hovering over Shabni, who is now burning in flames. Shabani eventually dies, causing the Third Child to focus on The Man on Fire once again, thus making him move again. Snake makes some distance between him and The Man on Fire and Miller Shouts over the radio and tells Snake to get out of there. Snake quickly evades from The Man on Fire and as Snake leaves the building, it explodes, surrounding the area in fire. Snake makes a break for the tunnel he previously entered, but The Man on Fire shoots it down, blocking Snake's escape. Snake fends off The Man on Fire and quickly escapes on Pequod. Snake returns to Mother Base, only to find the boys sitting by the landing zone. Snake gives one of the boys Shabani's Necklace so they can remember him.
